How did borders change in Europe in 1945-1946? Which boundaries established during this period have survived to our time, and which have changed?

This section of the new borders in Europe was defined by the Potsdam conference of the Big Three:
1) Germany lost all the territories seized by Hitler.
2) East Prussia withdrew from Germany to the USSR and Poland.
3) The Polish-German border was moved westward to the Oder and Neisse rivers
